Affinity / PCN1 & PCN10 Social Prescribing

Our Affinity/PCN1 and PCN10 service works much in the same way as CLICS, in that we receive referrals from GP practices, and then work with the referred service user on a one-to-one basis, tailoring any support and advice to their individual needs.
