MAST is a team of Voluntary Community Sector organisations based across Bradford Royal Infirmary (BRI) and Airedale General Hospital (AGH). MAST delivers interventions and health messaging with patients .

• HALE & KHL – Social Prescribing

 

Aleema and Hazel, our social prescribers work with patients who have accepted a community referral for social prescribing support on discharge. Providing up to 12 support sessions they will work together on identifying the patients’ needs and provide information and support enabling the patient to access relevant community services which are available to them.
