A Social Prescriber provides short term interventions for people who are isolated and experience low level mental health.

This role will support clients who have social, emotional or practical needs, thereby improving their health and wellbeing, in GP practice-based clinics sessions and remote/community settings

You will work closely with GP Practices and health professionals taking referrals and ensuring a joined-up approach to health and social care.
