Affinity / PCN1 & PCN10 Social Prescribing

Our Affinity and PCN1 Social Prescribing offer works on a 2 tier basis, with clients either receiving Tier 1 support through appointments in clinic (these may be in person in the GP Practice or over the ‘phone), or through a referral either from the GP or from one of our Social Prescribers. We work with the referred service user on a one-to-one basis, tailoring any support and advice to their individual needs.

The following practices are covered under this service:

• The Saltaire & Windhill Medical Partnership, and Idle Medical Centre.
• Cowgill Medical Practice, Sunnybank Medical Practice, Thornton-Denholme Medical Practice, The Willows Medical Centre, Shipley Medical Practice, and Haigh Hall Medical Practice.

 

In PCN10 we currently offer a Tier 1, clinic based, service.

The following practices are covered under this service:

• Leylands Medical Centre, Heaton Medical Practice, Ashwell Medical Centre, Hollyns Health & Wellbeing, and Manor Medical Practice.
