HALE Community Connectors

Our HALE Community Connectors work with GP Practices to deliver Social Prescribing in the following areas:

• Saltaire and Windhill, Idle, Cliffe Avenue Medical Practices.
• Affinity Partnership Practices, including Shipley, Haigh Hall, North Street, Sunnybank, Thornton, Cowgill and Denholme, Westcliffe and the Willows.
• Leylands, Heaton, Wrose, Ashwell, Hollins and The Manor Medical Practices

Each area has slightly different service offer, however referrals for all services are made directly through the GP Practice.
