What is HOP?

The Hypertension Outreach project is an exciting new project funded by the NHS to test and diagnose high blood pressure in people throughout the community and offer healthy lifestyle advice in order to enjoy lifelong good health.

Why have we set it up?

 We understand that approximately 32,250 people with the Bradford Districts CCG have hypertension but are currently undiagnosed. Hale in partnership with the BHF and Bradford Districts CCG are fully committed to ensuring a difference is made to the population of Bradford and are taking action to improve prevention, detection and management of Hypertension.

Therefore our aims are to:

  1. Include and involve larger numbers of people in our blood pressure testing service, particularly those who may be at risk due a combination of poor lifestyle factors, illness and disease.
  2. Be able to advise the public on adherence to healthier lifestyles and offer referral to professional medical services if required.
  3. Encourage feedback to improve service delivery plans and future decisions about healthcare services across the Bradford District.

By involving people in decisions about their healthcare, people’s overall health and wellbeing will improve.

Who is involved?

HOP is a partnership between:

HALE (Health Action Local Engagement) – A Bradford based healthy living charity working with the most vulnerable people in the community.

Bradford CCGs (NHS Clinical Commissioning Groups) – There are 3 CCGs working across the Bradford and Airedale District that plan, buy and monitor local health services with a clear vision of better health for the people of Bradford.

British Heart Foundation

We are the nation’s heart charity and the largest independent funder of cardiovascular research. Their pioneering research has helped to transform the lives of people living with heart and circulatory conditions.

 This partnership will work together to reach out to, involve and include the people of Bradford so that they can have easy access to our service.

Where will HOP team be?

You can find the HOP Team out in the community e.g. visiting existing community groups, attending community events, workplaces visiting places where people go.

We will also be online through our Facebook page and Twitter social media accounts where you can talk to us, share your views and ideas.

How can you get involved?

We want to hear from and listen to as many people living in Bradford who want to take part in the service or wish to have the team on site at a workplace or attending an event. You can get involved by calling the team on 01274 271088 or 07398351792 and following our social media accounts.

You can also get involved by volunteering with the HOP Team and helping us reach out to other people within your community.
