Salary ranging from £22,183 – £24,491 pro rata plus 5% pension 

One position – 16hrs per week

The Social Prescribing approach recognises that people’s health is affected by a range of social, economic and environmental factors and that linking people in with their community is highly effective in reducing health inequalities and increasing resilience.

A Social Prescribing Worker provides short-term interventions for individuals requiring support to access community services and activities, for reasons including isolation, low level mental health problems, or those who have had a sudden crisis.

You will work closely with GP Practices and health professionals taking referrals and ensuring a joined up approach to health and social care.

The successful candidate will also have excellent interpersonal skills and the energy and persistence to see tasks through to successful completion.
